All fees are due at the time of tryouts and should be provided in the form of a check made payable to "QV Hockey".
- $100 Non-Refundable Tryout Fee
- $300 Commitment Fee
If the player accepts an offer to play for QVHA, the full $400 will be applied to the season fees.
If the player declines an offer to play for QVHA, the $300 commitment fee will be returned.
Each player must complete the following forms which enable us to register you on a QVHA team. Please bring the following completed forms to tryouts.

- QVHA Zero Tolerance Agreement
- QVSD Chemical Use and Co-Curricular Activities Policy
Second-year Pee Wee players will also need to bring the following forms.
- PIHL Liability Waiver
- Copy of Birth Certificate
If you are transferring into the Quaker Valley School District, check with our registrar, Jackie McHenry, for additional documentation that is needed.
Each parent must complete/sign the SafeSport Receipt & Acknowledgement. By signing, you indicate that you have received and reviewed the following items.

All QVSD athletes need a signed PIAA Physical before they are allowed to participate in school sports. Use the link below to download your PIAA Physical form. If you plan on participating in ANY sport at QVSD, you MUST HAVE a completed physical turned into the school. You may drop off your completed forms to our Registrar, Jackie McHenry, or to the school nurse. Sports physicals must be completed prior to the first day of practice, but NOT BEFORE June 1st.
NO PLAYER is allowed to practice without having turned in a physical. Please keep a copy of your forms as backup.

IMPact Baseline Testing – QVHA’s policy is that students should have an IMPact test every two years. More information can be found here: https://www.qvsd.org/page.cfm?p=4729
All players must have a current IMPact test completed before they can represent the school at any game.
